I bought a headstall while over in the USA and I use it daily with belligerent horses in Holland. It can be harsh if used incorrectly but in saying that so is using a nose rope.
I do not trim or shoe without it.

I gave one to a customer of mine and the horse blewup from the harshness of it.
The horse reared and came down on this customer and all he saw was hooves dancing around his head. He ended up with a concusion.
In a qualified/experienced THINKING horseman this can be a good device. In the wrong hands it can be extremely severe.
